Job summary

AccentCare Inc. in San Diego County seeks an RN After Hours On Call Hospice Nurse (Weekends) to provide weekend on-call coverage with Friday 5pm-8am, Saturday 8pm-8am, and Sunday 8pm-8am shifts.

Pay is $55-$60 per hour plus mileage, based on experience. This role requires active RN licensure in the applicable state, with a background in hospice, home health, or related fields, and a willingness to participate in an on-call rotation.

Qualifications

• Current RN licensure in the applicable state is required; associate degree preferred.
• CPR certification is recommended (required for direct patient care in Georgia).
• Prior hospice, home health, medical-surgical, or geriatrics/pediatrics experience preferred.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families and the care team to develop and evaluate care plans.
• Maintain timely documentation and ensure care plans reflect current patient status.
• Provide education and emotional support to patients and caregivers and participate in on-call rotations and admissions.
• Coordinate all aspects of patient care when serving as Care Manager, including scheduling, medication management and IDG collaboration.
• Respond to triage calls and attend IDG meetings when in On-Call capacity; support training and other licensure scope duties.
