Gentiva in Raleigh, NC is seeking an experienced Hospice Clinical Liaison for weekends only. You will ensure intake and referrals are processed efficiently, coordinate admissions, and support community awareness of hospice services.

You will build strong relationships with hospitals, physicians, and discharge planners. The role requires an RN license with 3–5 years hospice experience (BSN preferred), CPR, and a valid driver's license.

Qualifications

• Licensed RN with 3–5 years hospice experience.
• BSN with 5+ years hospice experience preferred.
• Discharge planning, case management, or educator background preferred.

Responsabilités

• Receive and respond to referrals from hospitals and other professionals.
• Assess patient eligibility by reviewing records and communicating with physicians.
• Educate patients and families about hospice care and its value.
• Coordinate intake and admissions ensuring regulatory requirements are met.
• Liaise between referral sources and hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to support discharge planners.
• Build relationships with referral partners and identify outreach opportunities.
