Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in home and community settings. You will lead nursing assessments, coordinate care with the interdisciplinary team, and educate families to support comfort at end of life.

The role requires CT licensure, 1 year of nursing experience (or 1 year in a specialty), and a BSN is preferred. Supervisory duties for LPNs and aides are included as part of care delivery.

Qualifications

• Graduate of an RN program (Associate's or Diploma) and CT licensure.
• Experience: 1 year nursing or 1 year in a specialty; licensure to practice in CT.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and documents plan of care.
• Develops and updates care plans with client and interdisciplinary team.
