Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice Registered Nurse to provide skilled nursing care to hospice clients in home and community settings. You will assess patients, develop and implement care plans, and coordinate with the interdisciplinary team.

The role involves supervising LPNs and Home Care Aides and supporting families through end-of-life care. The position requires CT licensure and at least one year of nursing experience, with preferred BSN.

Qualifications

• Education: Associate’s diploma in nursing.
• License to practice in the State of Connecticut.
• One year nursing experience or one year in a specialty.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and documents findings.
• Develops and modifies care plans with client and team.
• Utilizes hospice levels of care and coordinates with providers.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Provides patient/family teaching and bereavement support.
• Refers clients to community resources as needed.
