Qualified candidates hold an Associate's Degree in Nursing (BSN preferred), CT license, and at least one year of nursing experience, ideally in home health or hospice.

Qualifikationen

• One year nursing experience required or one year nursing experience in a specialty.
• Competence in home care or hospice settings preferred.
• Connecticut licensure required.

Aufgaben

• Provides skilled nursing care to hospice clients in their homes or community settings.
• Reviews referrals and initiates contact with client and family.
• Performs comprehensive nursing assessments and documents care needs.
• Develops and updates the plan of care with client and interdisciplinary team.
• Facilitates peace at end of life and bereavement follow-up.
• Leads LPNs and supervises Home Care Aides in carrying out the care plan.
• Coordinates care with physicians and community resources.
• Participates in ongoing education and program evaluations.
