Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in home and community settings across Connecticut. The role emphasizes assessment, symptom management, and collaboration with the interdisciplinary team to plan and deliver compassionate care.

The ideal candidate holds an Associate’s or Diploma in Nursing with CT license, and at least one year of nursing experience (or hospice/home care experience).

Qualifications

Graduate of an Associate’s or diploma program in nursing.
CT licensed to practice in the state of Connecticut.
One year nursing experience required; or one year nursing experience in a hospice/home care specialty.

Responsibilities

Review referrals and initiate contact with client and family.
Conduct comprehensive nursing assessments and document care plans.
Coordinate interdisciplinary care and discharge planning; manage symptoms and end-of-life care.
