Job summary

Hartford HealthCare at Home is seeking a Hospice RN for weekend shifts. The role involves providing skilled nursing care to hospice clients in their homes or community settings, ensuring high-quality, cost-effective care.

The RN will assess patient needs, develop care plans with the client and interdisciplinary team, and coordinate services across providers. Responsibilities include reviewing referrals, documenting care, guiding LPNs and aides, and educating families about end-of-life care.

Qualifications

• Graduate of an Associate’s or diploma program in nursing.
• One year nursing experience or one year nursing experience in a specialty.
• License to practice in the State of Connecticut.
• BSN preferred; home care or medical/surgery experience preferred.

Responsibilities

• Review referrals and initiate contact with client and family.
• Conduct comprehensive nursing assessments based on referral data and patient status.
• Develop a plan of care with client and interdisciplinary team; document goals and utilization of resources.
• Document and maintain patient records according to agency standards.
• Coordinate care with physicians and other providers; utilize the interdisciplinary team.
• Direct LPNs and Home Care Aides in carrying out the plan of care.
• Refer clients/family to community resources as needed.
• Provide patient and family education on end-of-life signs and comfort measures.
