Constellation Health Services is seeking a Saturday/Sunday Registered Nurse with a focus on Hospice Care. The role will cover patients at Greenwich Hospital, Stamford Hospital and Norwalk Hospital. Shifts are eight or ten hours on Saturdays and Sundays.

The RN will provide skilled nursing care, coordinate care with the team, and document in the agency system. Qualifications include CT RN licensure and 1–2 years of RN experience, with hospice care strongly desired.

Qualificações

• Graduate of an accredited RN program.
• Registered Nurse licensed in CT.
• At least 1–2 years of RN experience.
• Hospice care strongly desired.

Responsabilidades

• Provides skilled nursing care including medications and treatments as prescribed.
• Performs comprehensive assessments and coordinates the plan of care with all disciplines involved.
• Observes and reports changes in patient's status to nursing supervisor and physician.
• Participates in weekly Interdisciplinary Team Meetings and updates on patient status.
• Maintains documentation per agency standards and uses the electronic system.
• Provides input for hospice aide evaluation as needed.
• Weekend and holidays coverage as needed.
