Job summary

Hartford HealthCare at Home is seeking a Primary Care Nurse for a weekend home health/hospice role in Norwich, CT. The RN will assess patients, plan and provide skilled nursing care in clients' homes, and collaborate with the IDT to adjust plans as conditions change.

Qualifications

• Graduate of an Associate’s degree or diploma program in nursing.
• One year nursing experience required; home care or hospice preferred.
• Current CT nursing license.

Responsibilities

• Provide skilled nursing care to hospice clients in homes or community health settings.
• Conduct comprehensive nursing assessments based on referrals and patient/family interviews.
• Develop and revise care plans with client and multidisciplinary team.
• Coordinate care, initiate referrals to services, and document care accurately.
• Deliver patient and caregiver education to promote comfort and safety.
• Supervise and direct LPNs and home care aides as assigned.
