Job summary

Beth Israel Lahey Health is seeking a Weekend Baylor Hospice RN to plan and deliver compassionate home-based end-of-life care. You will assess, plan, implement, and evaluate nursing care, coordinating with the interdisciplinary team to meet patient and family needs.

The role emphasizes symptom management, teaching, and family support, with weekend and on-call duties and collaboration with community resources. Requires NH RN license and BLS.

Qualifications

• Active New Hampshire RN license required.
• American Heart Association BLS certification required.
• Associate’s Degree or BSN from an accredited school of nursing preferred.
• At least 1 year of direct care nursing experience; OASIS/EPIC experience preferred.

Responsibilities

• Plan and deliver nursing care using the nursing process.
• Provide palliative and hospice care in patient’s home.
• Coordinate care with interdisciplinary team and community resources.
• Teach, counsel and support patients and families.
• Supervise Hospice Aides and document care accurately.
• Participate in weekend/holiday/on-call rotations.
