Job summary

Beth Israel Lahey Health in Massachusetts is seeking an Evening Hospice RN to join our growing hospice team. This role is Monday through Friday, 1:00pm–9:00pm, delivering compassionate, patient‑centered care to support patients and families at the end of life.

Responsibilities include completing initial and ongoing assessments, developing patient‑specific plans of care, managing pain and symptoms, teaching families about medications, and supervising Hospice Aides.

Qualifications

• Associates Degree or BSN (preferred) from an accredited school of nursing;
• An active valid Massachusetts RN license;
• An active American Heart Association BLS certification;
• At least 1 year of direct care nursing experience, preferably public health or home care nursing;
• EPIC experience is preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine hospice care needs.
• Perform a complete physical assessment of patient and history of current and previous illness(es); provide information to patient and family regarding the dying process and expectations.
• Develop a patient-specific plan of care, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and palliative nursing actions with emphasis on pain and symptom management.
• Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency.
• Provide documentation of services utilizing problem-oriented record.
• Instruct and supervise care given by Hospice Aides; supervise the Hospice Aide every two weeks minimally or as needed.
• Participate in bi-weekly hospice case conference or as required.
