Job summary

Beth Israel Lahey Health is seeking an experienced Hospice RN Team Resource for an evening shift (4:00pm-9:00pm, Monday through Friday) in Massachusetts. You will assess patients and families, develop care plans, and coordinate with the interdisciplinary team to deliver compassionate end-of-life care.

Requirements include an active Massachusetts RN license, BLS certification, and at least 1 year of direct nursing experience in public health or home care.

Qualifications

• Active Massachusetts RN license.
• BLS certification from American Heart Association.
• At least 1 year of direct care nursing experience, preferably in public health or home care; EPIC experience preferred.

Responsibilities

• Complete and document initial assessment of patient and family to determine hospice care needs.
• Perform comprehensive physical assessment and history of illnesses.
• Develop patient-specific care plans with goals based on nursing diagnosis.
• Educate patient and family about the dying process and medication actions.
• Document services using problem-oriented records.
• Supervise Hospice Aides and review care every two weeks or as needed.
• Participate in bi-weekly hospice case conferences.
