Job summary

Beth Israel Lahey Health is seeking an experienced Hospice RN to join a dedicated interdisciplinary team. You will develop patient-specific plans of care driven by patient goals and preferences, and serve as the eyes and ears of the physician in the home or facility to guide treatment for end-of-life pain and symptoms.

You will coordinate the plan of care, maintain EMR documentation, and educate patients and families on medications and care processes to maximize comfort in diverse settings.

Qualifications

• Current Massachusetts professional RN license.
• Baccalaureate degree in nursing from an NLN accredited program preferred.
• Recent Hospice Case Management or related Home Health Experience required.
• Expertise in EMR (PTCT) required hospice documentation.
• Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management.
• Demonstrated ability to assess and respond to needs of patients and families in a variety of settings.
• Valid Massachusetts driver’s license and access to reliable transportation.
• Ability to read, write and speak English.
• Comprehensive knowledge and skills of current end of life nursing practices.
• Skilled in interdisciplinary team approach to care and nursing process.
• Ability to communicate effectively with people from a variety of backgrounds.
• Ability to sustain positive attitude and personal connections despite exposure to continual loss.

Responsibilities

• Provide care according to hospice philosophy and develop patient-specific plans of care.
• Act as a member of the interdisciplinary team and coordinate care with patients and families.
• Communicate accurate patient assessments to the physician and guide treatment for end-of-life pain and symptoms.
• Perform RN pronouncement in all settings except acute care hospitals.
• Prepare and update the patient care plan and provide updates every 14 days or before recertification.
• Coordinate all aspects of the patient’s plan of care and liaise with physicians, pharmacists, and IDG staff.
• Educate patients, families, and contracted caregivers on medications and care processes to maximize comfort.
• Document care issues and goals in the EMR for quality indicators and timely sharing with clinicians.
• Attend to quality and performance improvement activities and participate in in-service training.
