Job summary

Beth Israel Lahey Health is seeking an Evening Hospice RN for two weeknights per week, 4:00pm-9:00pm. The role focuses on delivering comprehensive hospice nursing care, patient assessments, and care planning in collaboration with an interdisciplinary team.

The RN will perform assessments, develop care plans, educate patients and families, and supervise Hospice Aides while ensuring quality and compliance with standards. Massachusetts licensure and BLS certification are required.

Qualifications

• Initial patient/family assessment to determine hospice needs.
• Develop patient-specific plan of care with goals based on nursing diagnosis.
• Provide information to patient and family about dying process and expectations.
• Instruct patient/family on medication actions, side effects and dosing.

Responsibilities

• Complete and document an initial assessment of patient and family.
• Perform a complete physical assessment of patient and history of current and previous illness(es).
• Provide information to the patient and family regarding the dying process and expectations.
• Develop a patient-specific plan of care with goals based on nursing diagnosis.
• Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency.
• Provide documentation of services utilizing problem-oriented record.
• Instruct and supervise care given by Hospice Aides; supervise the care provided every two weeks minimally.
• Participate in bi-weekly hospice case conference.
