Job summary

Beth Israel Lahey Health at Home is seeking an Evening Hospice RN for a Monday–Friday 4:00pm–9:00pm schedule. You will perform assessments, develop plans of care, and educate patients and families on hospice care.

The role includes supervising Hospice Aides and participating in bi-weekly case conferences. Requirements include an active Massachusetts RN license, a BLS certification, and at least 1 year of direct care nursing experience, preferably in home care.

Qualifications

• Massachusetts RN license active in good standing.
• BLS certification active.
• At least 1 year of direct nursing experience, preferably home care.
• EPIC experience preferred.

Responsibilities

• Plan and deliver hospice nursing care using the nursing process.
• Perform initial and ongoing patient assessments and documentation.
• Educate patient and family on care plans, medications, and expectations.
• Supervise Hospice Aides and coordinate with the interdisciplinary team.
