Job summary

LiveWell Homecare Agency, LLC seeks a Registered Nurse to deliver patient-centered hospice care on a weekend-only schedule. You will assess physical and psycho-social status and develop care plans with the Interdisciplinary Group to support patients and families.

You will educate patients, manage pain and symptoms, ensure supplies and medications are available, maintain accurate documentation, and participate in IDG meetings.

Qualifications

• Minimum of one year of RN experience.
• CPR certification required.
• Reliable transportation, valid driver’s license, and insurance.

Responsibilities

• Conduct ongoing assessments of patients’ physical and psycho-social status.
• Develop and adjust care plans collaboratively with the Interdisciplinary Group, patients, families, and physicians.
• Educate patients and families, manage pain and symptoms, and evaluate responses to treatments.
• Ensure availability of necessary equipment, supplies, and medications.
• Maintain accurate documentation and participate in IDG meetings.
