Job summary

Superior Hospice, part of the Home Health Care, Inc. family, seeks a Weekend Hospice Visit Nurse to deliver expert clinical care in patients’ homes, with the option to work weekends based on availability, and to be supported by an interdisciplinary team.

You’ll provide one-on-one hospice care, coordinate with clinicians, and document visits in the EMR, while enjoying mileage reimbursement and competitive hourly pay. This role emphasizes meaningful weekend work and patient comfort.

Qualifications

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred, or experience in oncology, palliative care, ICU, home health, or end-of-life care.
• Strong assessment, communication, and critical thinking skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.

Responsibilities

• Complete routine and PRN hospice visits throughout your assigned territory.
• Assess patients, manage pain and symptoms, provide wound care, and respond to changes in condition.
• Educate and support patients, caregivers, and families with compassionate communication.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
