  • Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods relevant to hospice care.
  • Strong assessment skills combined with the ability to manage acute pain and complex symptom control effectively.
  • On-call shifts required
  • Prior experience in home care or community-based nursing environments is required; Med-Surg, long-term care or nursing home experience is beneficial.
  • Ability to work independently during weekends while maintaining excellent communication with the broader healthcare team.
  • Familiarity with infection control practices and aseptic technique is essential for safe medication administration and procedures like wound or catheter care.

  • Administer medications and treatments such as IV infusions, pain management protocols, and wound care, ensuring adherence to hospice standards.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with patient wishes and clinical needs.
  • Provide education and emotional support to patients and their families, guiding them through complex medical decisions and end-of-life processes.
  • Maintain accurate medical documentation within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
  • Respond promptly to urgent patient needs via triage protocols, coordinating with emergency services when necessary.
  • Participate in ongoing training related to hospice & palliative medicine, infectious disease care, and advanced symptom management techniques such as opioid titration or ostomy care.
