Job summary

Sterling Home Health, Llc is seeking an experienced Registered Nurse to provide 24/7 nursing coverage for patients, families, and community partners. Roles include in-person visits, hospice admissions, and after-hours symptom support.

You will work within licensure requirements, coordinate with case managers, and participate in hospice care implementation while ensuring patient safety and quality of care.

Qualifications

• Must be licensed as a Registered Nurse in North Carolina.
• Knowledge of Federal, State and Local regulations including Medicare COPs.
• HIPAA privacy rules and regulations knowledge required.
• Experience with electronic medical records systems preferred.

Responsibilities

• Assist with providing 24/7 nursing coverage via phone and in-person visits, including hospice admissions.
• Practice within the scope of license and regulations.
• Serve as first responder for after-hours symptom-based calls from patients.
• Provide guidance to patients and families, escalate to case management as needed.
• Participate in coordinating and implementing hospice care.
• Assist with emergency admissions and clinical assessments.
• Assess patients' physical and mental health and document findings.
• Plan, initiate and evaluate nursing actions and treatments.
• Teach, delegate and supervise staff in implementing care plans.
• Collaborate with health care providers for patient/family plans.
• Administer prescribed regimens and monitor responses.
• Provide patient education and counseling on health.
