Job summary

Four Seasons Care is seeking an experienced RN to provide 24/7 nursing coverage for patients and families, including hospice admissions when needed. You will work within licensed scope, respond to after-hours calls, and guide care planning in collaboration with the case manager and other providers.

The role requires knowledge of HIPAA, EMR systems, and regulatory requirements, with strong communication skills and the ability to manage multiple tasks under pressure.

Qualifications

• Must be willing to care for patients with infectious disease processes unless approved accommodations.

Responsibilities

• Assist with providing 24/7 nursing coverage via phone & in-person visits, including hospice admissions as needed.
• Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.
• Serve as first responder for after-hours symptom-based calls from current patients.
• Provide guidance to patients and families and coordinate with case manager follow up.
• Participate in implementation and coordination of patient’s hospice care.
• Assess the patient's physical and mental health and document nursing assessments.
• Plan, initiate, deliver, and evaluate nursing acts.
• Teach and supervise others implementing the treatment regimen.
• Collaborate with other health care providers on patient and family-centered plan.
• Implement prescribed regimen and provide teaching about health.
