Job summary

Longleaf Hospice and Palliative Care is seeking an Admission Nurse to perform consultation and admission visits, assess patient and family needs, and collaborate with physicians to determine hospice eligibility. This role requires coordinating with the IDG and Case Manager, developing initial and updated care plans, and providing counseling to patients and families.

Qualifications

• Current license to practice as a registered professional nurse in Georgia.
• Current CPR certification.
• At least two years of recent hospice experience.
• Excellent verbal and written communication and problem-solving skills.
• Valid driver’s license; reliable, insured, automobile for work travel.
• Must be willing to complete admissions throughout Longleaf Hospice's entire service area.

Responsibilities

• Collaborate with the patient’s attending physician and the Hospice physician to determine clinical eligibility for hospice care.
• Assess patient’s medical status, plan, teach and implement immediate medical interventions on day of admission as prescribed by physician’s orders.
• Participate in coordination of all aspects of patient’s care with current professional standards and practice.
• Participate in ongoing interdisciplinary comprehensive assessments as needed.
• Develop the initial Plan of Care (POC) and updates in collaboration with the IDG, attending physician, Hospice physician, and patient/family.
• Contribute to patient and family counseling and education.
• Participate in QAPI and Hospice in-service activities.
• Coordinate medications and DME needs for same-day delivery.
• Complete all elements of admission process and first 24 hours of care.
• Communicate admissions information and concerns in meetings to ensure smooth transition with IDG staff and Case Manager.
