Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for the Richmond area. You will perform Start of Care admissions and SOC/OASIS, conduct follow-ups, and visit patients at home to assess needs and plan care.

Responsibilities include medication reconciliation, wound care, IV infusions, and coordinating with physicians and the clinical team. A CA RN license, BLS, and valid CA driver’s license are required.

Qualifications

• Active CA RN license required.
• BLS/CPR certification required.
• Experience with Start of Care (SOC) and OASIS is highly desired.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow-up and evaluation visits in the designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments and procedures.
• Monitor clients’ conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver high-quality care.
• Document observations and interventions concisely.
• Initiate IV medication infusions per protocol.
