Job summary

Innovive Health is seeking a Weekend Registered Nurse (RN) to partner with the Clinical Team in delivering skilled nursing care to home health patients. This full-time exempt role requires coordinating care, administering medications, and maintaining documentation in accordance with physician orders and company policies.

Weekend hours are Saturday and Sunday 7:00 AM–8:00 PM, plus four hours on Mondays for follow-ups and documentation.

Qualifications

Graduate of an accredited school of professional nursing.
Licensed RN in the state; valid license.
Passed the NCLEX examination.
Maintains professional standards and licensure.
Has reliable transportation and valid driver’s license.

Responsibilities

Provide direct care and case management for assigned patients in home health.
Assess patient status, revise plans of care, and communicate changes to the care team.
Manage medications, coordinate refills with physicians and pharmacies.
Educate patients and families on self-care, meds, and diet per physician orders.
Ensure a safe home environment and report safety concerns.
Supervise Home Health Aide and LPN performance and care quality.
Perform specialized procedures when trained and competent (e.g., IV therapy).
Complete clinical notes and OASIS documentation per policy.
