Job summary

Innovive Health is seeking a Weekend Registered Nurse (RN) for home health. The role involves direct patient care, case management, and collaboration with the Clinical Team to follow physician orders and ensure compliant, high-quality home-based nursing services.

The Weekend Baylor RN schedule runs every weekend (Sat/Sun, 7:00 AM–8:00 PM) with four hours on Mondays for follow-ups and documentation; this is a full-time exempt position.

Qualifications

• Graduate of an accredited school of professional nursing.
• Licensed RN in the state (current licensure).
• Must have passed the NCLEX exam.
• CPR certification required.
• Must have a valid driver’s license and reliable transportation.
• Pre-employment health clearance including TB test (or CXR) per policy.
• Satisfactory professional references from nursing school or prior employers.

Responsibilities

• Provide direct care and case management for assigned patients in home health, including initial assessment and plan of care.
• Administer medications and treatments as prescribed; maintain medication profiles and coordinate refills.
• Monitor patient status, adjust care plans, and communicate changes to the team and physician.
• Educate patient and family on self-care, medications, and health counseling.
• Ensure HIPAA compliance and maintain patient confidentiality.
• Supervise Home Health Aide & LPN; perform field supervision as needed.
• Complete accurate clinical notes and OASIS documentation per policy.
• Coordinate with physicians, pharmacies, and insurers to support timely care.
• Participate in case management meetings and care coordination meetings.
