Job summary

Innovive Health is seeking a Weekend Registered Nurse to partner with the clinical team in delivering skilled nursing care to home health patients. This full-time exempt role covers Saturdays and Sundays from 7:00 AM to 8:00 PM, with four hours on Mondays for follow-up and documentation.

You will assess patients, develop and update plans of care, manage medications, educate families, and coordinate with physicians and other providers to ensure safe, compliant, high-quality home health care.

Qualifications

• Licensed RN in the state where the position is located
• Bachelor's degree in nursing or diploma/associate with required experience
• Must have NCLEX and current CPR certification
• Must have a valid driver's license and reliable transportation
• Pre-employment health clearance and satisfactory references

Responsibilities

• Provide direct care and case management for assigned home health patients
• Develop, update, and execute plans of care
• Administer and monitor prescribed medications and treatments
• Coordinate schedules and communication with physicians and care teams
• Educate patients and families on self-care and safety
• Maintain accurate clinical documentation and OASIS as required
• Ensure HIPAA compliance and patient confidentiality
