Job summary

Innovive Health is seeking an experienced Registered Nurse for weekend home health coverage. The Weekend Baylor RN will work Saturdays and Sundays from 7:00 AM to 8:00 PM, plus four hours on Mondays for follow-ups and documentation, as a full-time exempt role.

The RN will deliver direct patient care, develop and adjust care plans, administer medications, teach patients and families, and coordinate with physicians to ensure safe, effective home health services.

Qualifications

• Graduate of an accredited nursing program and licensed RN in the relevant state.
• Experience in home health or skilled nursing settings preferred.
• Strong assessment, care planning, and patient/family education skills.
• Ability to work independently within a clinical team and meet documentation standards.

Responsibilities

• Provide direct patient care and case management for home health patients.
• Assess patient status, develop and update plans of care, and coordinate treatments.
• Administer prescribed medications and monitor for adverse effects.
• Educate patients and families on self-care techniques and medications.
• Maintain HIPAA privacy and complete clinical documentation and OASIS as required.
• Collaborate with physicians, therapists, and other care team members to ensure continuity of care.
