Job summary

Innovive Health is seeking a Weekend Registered Nurse (RN) to partner with the Clinical Team to deliver skilled nursing care to home care patients on weekends. The role includes direct care, case management, documentation, and coordination with physicians and pharmacies.

The weekend schedule is Saturday and Sunday 7:00 AM–8:00 PM, plus four hours on Mondays for follow-ups and documentation. This is a full-time exempt position.

Qualifications

• Graduate of an accredited school of professional nursing.
• RN license in good standing with the State Board of Nursing.
• Must have CPR certification and NCLEX completion.
• Possesses and maintains valid driver’s license and insured vehicle.
• Excellent documentation and adherence to HIPAA guidelines.

Responsibilities

• Provide direct clinical care and case management for home health patients.
• Develop and update plan of care with physician orders and care team.
• Administer medications and treatments per physician orders.
• Monitor patient status and adjust plan of care as needed.
• Educate patient and family on self-care, meds, and safety.
• Maintain accurate medication profiles and documentation.
• Coordinate with pharmacies, providers, and other services.
• Perform field supervision and take on assigned duties as required.
