Job summary
Infirmary HomeCare, part of the LHC Group and Optum family, seeks a Weekend RN with home health experience to coordinate and deliver nursing care in patients' homes. Friday/Saturday/Sunday or Saturday/Sunday/Monday shifts are available, with a focus on professional, compassionate patient care and interdisciplinary collaboration.
The RN will assess, plan, and implement patient care, collaborate with physicians and staff, and participate in ongoing quality initiatives.
Qualifications
- Current RN licensure in Alabama.
- Valid driver's license and access to reliable transportation.
- CPR certification or ability to obtain within 90 days of hire.
Responsibilities
- Develops, implements, and updates patient plans of care with physicians and the interdisciplinary team.
- Provides and coordinates high-quality clinical care; supervises home health aides and LPN/LVN staff.
- Accurately documents all patient care per policy; communicates with physicians, patients, families, and staff.
- Participates in orientation and precepting; supports performance improvement and patient safety initiatives.
- Demonstrates professionalism, punctuality and adherence to agency standards; participates in on-call/weekend rotations.
- Ability to work in diverse home environments and with patients of varied backgrounds.
