Concierge Home Care in Sarasota, FL is seeking a Weekend Home Health RN to provide skilled nursing care in patients' homes during weekends. You will develop care plans, educate patients and caregivers, coordinate with physicians, and document visits with AI-assisted tools.

This role offers weekend autonomy and a pathway to build patient relationships. Active Florida RN license, valid driver's license, and home health experience are preferred.

Qualifications

• Active Florida RN license is required.
• Valid driver’s license and reliable transportation.
• Home health care experience preferred; training provided.

Responsibilities

• Conducting patient assessments and developing individualized care plans.
• Providing skilled nursing care and essential patient/caregiver education.
• Coordinating care with physicians and interdisciplinary teams.
• Monitoring progress and adjusting care for patients at home.
• Documenting visits using our AI-assisted charting tools.
