Job summary

Concierge Home Care in Sarasota, FL seeks an experienced Home Health RN for after-hours and on-call coverage. This part-time role focuses on evening and weekend visits, with scheduling from 4:00 PM to 10:00 PM, Monday through Friday as needed.

Qualifications

• Active Florida RN license (or Compact) required.
• Home health, urology, med-surg, hospice, or acute care experience preferred.
• OASIS documentation experience required; strong assessment/triage skills expected.

Responsibilities

• Complete skilled nursing visits during evenings and weekends.
• Start of Care assessments; Resumption of Care; PRN visits.
• Coordinate care with case managers and interdisciplinary teams.
