Qualifications

• Florida RN license is required.
• OASIS documentation proficiency is required.
• Home health care experience preferred.

Responsibilities

• Conduct in-home OASIS assessments, evaluations, and develop personalized treatment plans.
• Provide clinical oversight and guidance to LPNs.
• Collaborate with patients, caregivers, and the clinical team to deliver care.
• Utilize dictation services to streamline documentation and care time.
• Manage acute and chronic diseases.
• Administer and adjust medications as prescribed.
• Support post-surgical rehabilitation.
• Manage pain and provide wound care.
