Job summary

Concierge Home Care, located in St Augustine, FL, is seeking Home Health Weekend Registered Nurses (RN) to provide 1:1 patient care in the home with flexible scheduling and autonomy. This role supports patients in St Augustine and St Johns County, offering flexible weekend options and no hospital shifts.

Qualifications

• Active Florida RN license in good standing.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.

Responsibilities

• Perform patient assessments and develop individualized care plans.
• Provide skilled nursing care and patient education.
• Coordinate care with physicians and interdisciplinary teams.
• Monitor patient progress and adjust care plans as needed.
• Complete documentation using AI-assisted tools.
