Job summary

Concierge Home Care in Clearwater, FL is seeking a Weekend Home Health RN to deliver skilled nursing care in patients' homes. You’ll provide assessments, develop care plans, manage medications, and coordinate with physicians to ensure continuity of care during weekend hours.

Qualifications

• Active, unencumbered Florida RN license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience preferred, but we are happy to train the right candidate!

Responsibilities

• Conducting patient assessments and developing individualized care plans.
• Providing skilled nursing care and essential patient/caregiver education.
• Coordinating care with physicians and interdisciplinary teams.
• Monitoring progress and adjusting care for patients recovering at home.
• Documenting visits efficiently using our streamlined AI‑assisted tools.
