Job summary

CVHCare, a leader in Home Health Clinical services, seeks a Registered Nurse for weekend Home Health field visits in California. This non-remote role offers per diem scheduling with 12 visits per week and potential Thursday–Monday shifts, servicing the Canoga Park–Encino–LA area.

As a Home Health RN you will develop an individualized plan of care with a primary physician, function as case manager, and provide detailed wound, post-surgical, ostomy care and patient education.

Qualifications

• 1 year RN experience in acute, SNF, Home Health or Hospice preferred.
• Active CA Registered Nursing License required.
• Current CPR or BLS certification required.

Responsibilities

• Monitor patient health and develop an individualized plan of care with the physician.
• Serve as case manager and coordinate independent home health visits.
• Provide wound therapy, post-surgical care, and ostomy care management.
• Educate patients and families on disease management and care plans.
