Job summary

CVHCare in California is seeking a Home Health Registered Nurse to monitor patients and develop individualized care plans with physicians. Each RN serves as a case manager, delivering high-quality home care and patient education in the patient's home.

The role requires active CA RN license, 1 year of RN experience in acute, SNF, home health or hospice, and availability to work weekends. Training onsite in San Ramon and non-traditional schedules are noted.

Qualifications

• Minimum 1 year RN experience in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License required.
• CPR or BLS Certification.
• Reliable transportation required.

Responsibilities

• Monitor a patient’s health and, with a primary care physician, develop an individualized plan of care.
• Function as a case manager delivering independent, high-quality home health care.
• Provide patient education on disease management and skilled services such as wound therapy, post-surgical care, ostomy care.
