Job summary

CVHCare is seeking a Weekend Home Health RN (Field) to monitor patients in their homes and, with a primary care physician, develop individualized plans of care. This is a non-remote field position with options for Full Time (24 visits per week) or Part Time (18 visits per week), including a Thursday–Monday schedule; paid training is onsite in San Ramon.

Qualifications

• Minimum 1 year RN experience in home health, acute, SNF or hospice
• Active California Registered Nursing License
• AA, BSN or MSN degree
• Reliable transportation
• CPR or BLS certification

Responsibilities

• Monitor patients at home and develop individualized care plans with physicians
• Function as case manager and deliver quality home health care
• Provide disease management education and wound/ post-surgical care as needed
• Coordinate with internal team for medication reconciliation and orders
