Job summary

CVHCare, a leader in Home Health Clinical services, is seeking a Registered Nurse for field coverage in California. This on-site role offers the opportunity to work with a compassionate team, delivering wound therapy, post-surgical care, and patient education in the home setting.

Qualifications include 1 year RN experience and a CA RN license; AA/BSN/MSN degrees are noted. Full-time or part-time schedules are available with paid training in San Ramon.

Qualifications

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting.
• Must possess an active CA Registered Nursing License.
• AA, BSN, MSN.

Responsibilities

• Monitor a patient’s health and develop an individualized plan of care in partnership with the primary care physician.
• Function as a case manager and deliver quality healthcare in the patient’s home.
• Provide patient education on disease management and specialized services such as wound therapy and post-surgical care.
• Coordinate with internal support teams to facilitate documentation and medication reconciliation.
