CVHCare is seeking a Weekend Home Health RN (Field) to join our California team. This per diem role offers 12 visits per week in the Los Angeles area, with non-traditional Thursday-Monday scheduling and in-home patient care.

You will monitor patients, collaborate with physicians, and deliver wound, post-surgical, and ostomy care under supervision. Onboarding includes five days of training at our Los Angeles office.

Qualifications

• Minimum 1 year RN experience in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License.
• CPR or BLS certification valid.
• Reliable Transportation.

Responsabilités

• Monitor a patient’s overall health and develop individualized plans of care with the primary care physician.
• Function as a case manager delivering in-home nursing care.
• Provide patient education on disease management and self-care.
• Deliver wound therapy, post-surgical care management, and ostomy care management.

Title:Weekend Home Health RN (Field)
Schedule: Per Diem ( 12 visits per week)
* Non-traditional schedules are available, i.e., Thursday-Monday
Locations: Area #40 covers the following cities:North Hollywood, Sherman Oaks, Studio City, Sun Valley, Toluca Lake, Universal City, Valley Glen, Valley Village, Van Nuys
Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm and go to our Los Angeles office for onboarding

Position Summary:The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management.

We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.

Requirements:

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
• Must Possess an active CA Registered Nursing License
• AA, BSN, MSN
• Reliable Transportation
• Current CPR or BLS Card
