Job summary

CVHCare is seeking a Weekend Home Health RN (Field) to join our California team. The RN will monitor patients, collaborate with primary physicians, and develop individualized care plans, delivering high-quality home health services at patients’ homes with a flexible weekend schedule.

The role emphasizes wound therapy, post-surgical care, and ostomy care management while supporting independence with extensive internal operational support for tasks like medication reconciliation and MD signatures.

Qualifications

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License is required.
• Current CPR or BLS certification.

Responsibilities

• Monitor a patient’s health and, with the primary care physician, develop an individualized plan of care.
• Act as case manager and deliver quality healthcare independently.
• Provide patient education on disease management and wound care as needed.
