Job summary

CVHCare, a leader in Home Health Clinical services, is seeking a Weekend Home Health RN (Field) to join our California team. This on-site role requires providing patient-centered home health care and disease management education, with a schedule of 24 visits per week for full-time or 18 visits per week for part-time.

Training is on-site in San Ramon. Requirements include: CA RN license, minimum 1 year RN experience in home health or related settings, AA/BSN/MSN, CPR/BLS, reliable transportation.

Qualifications

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting.
• Must possess an active CA Registered Nursing License.
• AA, BSN, MSN.
• Reliable transportation.
• Current CPR or BLS Card.

Responsibilities

• Monitor patient health in partnership with a primary care physician and develop an individualized plan of care.
• Serve as case manager and deliver quality home health care.
• Provide patient education on disease management and specialized services as needed (e.g., wound therapy, post-surgical care).
• Coordinate with internal support team to pre-populate medication reconciliation and manage documentation.
