Job summary

CVHCare in San Ramon, CA, seeks a Registered Nurse for Home Health field work. This non-remote, on-site role offers full-time or part-time schedules and weekend openings across Dublin, San Ramon, Pleasanton, and surrounding areas.

You’ll serve patients in home settings with flexibility to arrange visits around commitments. You will assess health, develop individualized care plans with physicians, act as case manager, and provide education on disease management.

Qualifications

• Minimum 1 year RN experience in acute, SNF, Home Health or Hospice.
• Must possess an active CA Registered Nursing License.
• AA, BSN, MSN are acceptable qualifications.
• Reliable transportation required.
• Current CPR or BLS certification.

Responsibilities

• Monitor patient health and develop an individualized plan of care with the primary physician.
• Serve as a case manager and deliver quality home healthcare.
• Provide patient education on disease management and self-care.
• Perform specialized services such as wound therapy, post-surgical care, ostomy care as applicable.
