CVHCare, a leader in home health care, is seeking a Weekend Home Health RN (Field) to join our California team. This per diem role covers area #42 including Canoga Park, Chatsworth, Encino, and surrounding communities. The RN will provide in-home care with autonomy, visiting patients on a 12-visit weekly schedule.

Requires CA RN license and a minimum of 1 year RN experience. Training spans 5 days in our Los Angeles office, with non-remote field work and a competitive benefits package.

Qualifications

• Minimum 1 year RN experience in home health, acute, SNF or hospice settings.
• Active California Registered Nursing License.
• CPR or BLS certification.

Responsabilités

• Monitor patient health and partner with the primary care physician to develop an individualized plan of care.
• Function as a case manager delivering in-home healthcare independently.
• Provide patient education on disease management and wound, post-surgical, or ostomy care as needed.
• Coordinate with internal support team for medication reconciliation, DME orders, MD signatures, and lab follow-ups.
