Job summary

Health Care Connectors LLC is seeking a Baylor Registered Nurse (RN) to deliver skilled nursing care to home health patients on designated weekend hours. The role supports continuity of care through assessments, visits, admissions, and timely response to changes in patient condition.

The RN will collaborate with physicians, patients, families, and the care team to develop individualized care plans. Strong clinical judgment, effective communication, and independence in home settings are essential.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current Connecticut RN license or eligibility to practice in CT.
• At least 1 year of clinical nursing experience; home health experience preferred.
• Current CPR/BLS certification.
• Strong assessment, critical thinking, and care coordination skills.
• Excellent organizational, communication, and time-management skills.
• Ability to work independently and prioritize patient needs.
• Valid driver’s license and reliable transportation for patient visits.

Responsibilities

• Conduct comprehensive nursing assessments and identify patient care needs.
• Complete admissions, resumptions of care, re-certifications, routine visits, and discharges as assigned.
• Develop and update individualized care plans with patient, physician, and care team.
• Provide skilled nursing services including wound care and medication management.
• Monitor patient progress and communicate changes to providers and leadership.
• Educate patients and caregivers on medications, symptoms, disease processes and home safety.
• Complete accurate clinical documentation and required assessments.
• Coordinate with the interdisciplinary team to support weekend continuity of care.
• Provide clear hand-off communication and follow-up priorities.
• Adhere to infection prevention practices and agency policies.
• Participate in meetings, training, and quality improvement activities.
