Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to deliver skilled nursing visits and coordinate patient care across a multi-disciplinary team in the Arlington, VA area. You will educate patients and families, perform comprehensive assessments, and support home health services from admission through discharge.

Responsibilities include OASIS/485 changes, complex wound care, PICC line care, and collaboration with physicians.

Qualifications

• IL RN license, BLS within 30 days of hire
• Education: College Diploma in Nursing
• 1 year nursing experience required

Responsibilities

• Coordinate and provide direct and indirect patient care using nursing processes to meet physical and psychosocial needs
• Perform OASIS assessments and 485 changes within required timeframes
• Manage wound care, wound VACs, PICC lines and catheter/drain care per agency protocol
• Supervise Home Health Aide and coordinate care with interdisciplinary team
• Communicate with physicians and care coordinators to adjust plans of care
