Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing care in patients' homes. You will work within an interdisciplinary team to educate patients and families, manage complex wounds, IV infusions, and catheter care across multiple counties.

Qualifications

• IL RN license required and BLS certification.
• Valid driver's license and proof of auto insurance.
• College diploma in nursing; home care experience preferred.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform complex wound care including wound vac, PICC/ports, and dressing changes.
• Complete OASIS/485 changes within 24 hours of receipt.
• Collaborate with MD, care coordinators, and insurers for plan updates.
• Supervise Home Health Aide and coordinate team activities in patient homes.
