Job summary

Carle Health is seeking a Home Care Registered Nurse/Field Case Manager to deliver comprehensive patient care in community settings, coordinating plans with an interdisciplinary team. No day is the same, and you will lead complex wound care, PICC line management and infusion therapies in patients' homes.

You will perform OASIS assessments, manage admissions and documentation, supervise Home Health Aides, and ensure timely care coordination to optimize patient outcomes and independence.

Qualifications

• IL RN license required
• BLS certification within 30 days of hire
• Driver's license and proof of auto insurance
• College Diploma in Nursing and 1 year nursing experience

Responsibilities

• Perform OASIS admissions, recertifications, resumptions and discharges within policy timelines
• Complete admission documentation promptly and accurately
• Direct and supervise Home Health Aide staff and coordinate with the health care team
• Manage complex wound care including wound vacs, catheters and IV infusions as per protocol
• Coordinate care plans, educate patients and families, and ensure proper care transitions
