Job summary

AmeriCare is seeking a Visiting Nurse (RN Coordinator of Care) to manage home-based patient care. The RN will conduct home visits, complete documentation, and communicate with the PCP to ensure seamless care.

Full-time schedule includes Friday, Saturday, and Monday with opportunities for sign-on bonuses and strong benefits. Valid NYS RN license, reliable transportation, and independent work style are essential.

Qualifications

• NYS RN license is required.

• Valid driver’s license and reliable transportation are required.
• Ability to work independently with minimal supervision.
• Excellent clinical skills and patient care focus.
• Basic knowledge of Outlook, Word, and scanning tools.

Responsibilities

• Conduct home visits and complete required documentation.
• Coordinate with PCP and other members of the care team.
• Support patient education and care planning in the home setting.
