Visiting Nurse (RN Coordinator of Care)

The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This includes conducting home visits, completing all required documentation, and communicating with PCP.

Full‑time schedule: Monday‑Friday.

Compensation & Benefits
