Job summary

AMERICARE CERTIFIED SPECIAL SERVICES INC is seeking a Visiting Nurse (RN Coordinator of Care) to manage all patient care aspects in the home, including visits, documentation, and communication with the PCP. This is a full-time role with a Monday–Friday schedule.

We welcome new grads and caregivers; requires NYS RN license and a valid driver’s license. Benefits include sign-on bonus, tuition reimbursement, 401(k) with match, paid orientation, and company tablet with support.

Qualifications

Visiting Nurse (RN Coordinator of Care)

The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include Conducting home visits, completion of all required documentation and communication with PCP.

Full time schedule: Monday-Friday Full Time RN
