Job summary

AmeriCare is seeking a Visiting Nurse (RN Coordinator of Care) to manage home-based patient care. The RN will conduct home visits, complete documentation, and communicate with the PCP to ensure seamless care.

Full-time schedule includes Friday, Saturday, and Monday with opportunities for sign-on bonuses and strong benefits. Valid NYS RN license, reliable transportation, and independent work style are essential.

Qualifications

• NYS RN license is required.

• Ability to work independently with minimal supervision.
• Excellent clinical skills and patient care focus.
• Basic knowledge of Outlook, Word, and scanning tools.

Responsibilities

The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include

Conducting home visits, completion of all required documentation and communication with PCP.
